Steps to join the private practice
Become a Periodic, Quarterly or Long-Term Client

I look forward to working with you in the coming weeks. Before we can begin, I ask that you complete the practice entry forms below. After I’ve received your completed forms, you will be admitted as a ‘Periodic Client’ and can use the scheduling calendar to book available time slots. After our initial quarter of work together is complete, I will determine your booking preferences and availability. At this time, you can increase session frequency and pre-book appointments to ensure consistent sessions.

    Once you have heard from me, you can schedule your first session.
